On this page you will find templates for the forms and notices required to enable voters to vote in local elections in Wales on 3 May 2012.

The tables below are separated into documents for combined polls and combined issue, and documents for standalone polls or separate issue (at a combined poll). All documents are available as Word templates by clicking on the links in the tables below.

Poll cards, postal voting statements and postal voting guides 

Printers and designers can download generic InDesign files for each document type and insert the correct wording from the Word templates.
