Guidance
The Electoral Commission has designed a set of Voter ID forms to support Electoral Administrators and voters with the introduction of the new Voter ID requirements at polling stations from May 2023.
You should review the application form and letter templates together with our guidance for Electoral Registration Officers on processing Voter Authority Certificate and Anonymous Elector’s Document applications in England, Scotland and Wales.
Design
Most of the Voter ID forms are available in both colour and mono, and in three software formats; InDesign, Word and PDF. We recommend you use the InDesign templates where possible, since they best support the form layout, but both Word and PDF can be edited and printed.
The InDesign templates can be printed on A3 paper and folded to make an A4-size booklet, but the content and layout of the forms is not prescribed in legislation, so you do not need to follow this layout.
Editing the forms

The first page is the ‘letter’ part of the form. It contains information for the applicant about how to apply before they move to the ’doing’ part of filling out the form. You will need to edit the letter to add the applicant’s name and address, the date and your contact details and logo.
The back page contains guidance for the applicant. You will need to edit the back page to add your contact details and a link to your privacy statement.
