On the form you will be asked to state that you are qualified and not disqualified from standing. You must also state your date of birth.You are not allowed to sign the consent form earlier than thirty-one calendar days before the last day for submitting your nomination papers. Your signature must be witnessed, and the witness must sign the form and give their full name and address. There are no restrictions on who can be a witness to the consent to nomination. 1.
